PART I, LINE IA: THE ORGANIZATION PROVIDED A TAXABLE REIMBURSEMENT OF HEALTH OR SOCIAL CLUB DUES OR INITIATION FEES FOR UP TO $200 PER EMPLOYEE BASED ON A 50% REIMBURSEMENT.
PART I, LINE 7: DESCRIPTION OF NON-FIXED PAYMENTS DISCRETIONARY BONUSES ARE AWARDED BASED UPON THE EMPLOYEE MEETING A VARIETY OF PERFORMANCE METRICS. ALL BONUSES ARE REVIEWED BY THE CEO, SU2C CEO AND CFO. THE ONLY EXCEPTION IS THE CEO'S BONUS, WHICH IS DETERMINED BY THE BOARD OF DIRECTORS. ADDITIONALLY, THE PROPOSED BONUSES FOR ANY KEY EMPLOYEES AND OFFICERS ARE SUBMITTED TO THE EXECUTIVE COMMITTEE OF THE BOARD OF DIRECTORS FOR THEIR REVIEW AND APPROVAL.
