SCHEDULE J, PART I, LINE 4A: Karen Chupka received severance in the amount of: $572,187 Cindy Stevens received severance in the amount of: $169,141 Jean Foster received severance in the amount of: $164,952
SCHEDULE J, PART I, LINE 4B: GARY SHAPIRO WAS PAID $691,708 (TRUST II $303,876 AND TRUST III $387,832) UNDER A DEFERRED COMPENSATION PLAN. THE CONTRACTED PAYMENT IS BASED ON HIS CONTINUOUS EMPLOYMENT THROUGH 12/31/2010 FOR TRUST I, 12/31/2015 FOR TRUST II, 12/31/2020 FOR TRUST III AND 12/31/2023 FOR TRUST IV. ANNUAL DISTRIBUTIONS EQUAL 50% OF BASE SALARY AS OF 12/31/2010 AND 12/31/2015 PAID IN 12 EQUAL MONTHLY INSTALLMENTS. ANNUAL DISTRIBUTIONS EQUAL 25% OF BASE SALARY AS OF 12/31/2020 AND 15% OF BASE SALARY AS OF 12/31/2023 PAID IN 12 EQUAL MONTHLY INSTALLMENTS. SENIOR EXECUTIVES ARE ELIGIBLE FOR THE SPECIAL INCENTIVE PLAN (SIP). ELIGIBLE EMPLOYEES RECEIVE AN AMOUNT EQUAL TO THEIR ANNUAL CASH BONUS THAT WILL BE EARMARKED IN THE SIP.
