PART I, LINE 2: ENLACE CHICAGO CREATED A REQUEST FOR PROPOSALS AND ALL ORGANIZATIONS SUBMITTED A FORMAL APPLICATION. SUBGRANTS WERE AWARDED TO COALITION PARTNERS IN ALIGNMENT WITH THE OVERARCHING GRANT GUIDELINES. ORGANIZATIONS WERE REQUIRED TO SUBMIT PROOF OF 501(C)(3) STATUS AND GOOD STANDING WITH THE ILLINOIS SECRETARY OF STATE. THE INITIAL GRANT PAYMENT WAS CONTINGENT ON A SIGNED MEMORANDUM OF UNDERSTANDING (MOU) INCLUDING AN AGREEMENT ON OUTCOMES, SCOPE OF SERVICE, AND PROPOSED BUDGET. FURTHER PAYMENTS WERE CONTINGENT ON MOU COMPLIANCE, WHICH WAS CONFIRMED THROUGH QUALITATIVE AND QUANTITATIVE REPORTING SUBMITTED BY THE GRANTEE. INDIVIDUAL GRANTS WERE RELEASED AFTER AN APPLICATION HAD BEEN SUBMITTED TO A COALITION EXECUTIVE WHO THEN REVIEWED AND DETERMINED THE AWARD AMOUNT. INDIVIDUALS WERE REQUIRED TO SUBMIT FORM W-9S.
