What is the organization's primary exempt purpose?
To support people impacted by cancer in Southwest Colorado by offering social and emotional programming and services that improve quality of life and promote health, wellness, and healing. Our mission is to uplift and strengthen people impacted by cancer by providing this support, fostering compassionate communities, and breaking down barriers to care. Cancer Support Community's comprehensive programs are built around five pillars-emotional and social support, education, wellness, information assistance, referral and social connection.
Describe the organization’s program service accomplishments for each of its three largest program services, as measured by expenses. In a clear and concise manner, describe the services provided, the number of persons benefited, and other relevant information for each program title.

Community Outreach: Our website provides community outreach with educational and informational support at diagnosis and beyond by sharing assistance for the next steps, which decreases the fear and distress associated with any phase of a cancer diagnosis. In 2022 we had 28,449 visits to our website with 110,432 page views. Our outreach efforts are focused on building relationships that help us reach the people we need to serve in our rural communities of Southwest Colorado, which includes promoting awareness of recommended screenings for early cancer detection. All expenses incurred with these programs are related to a portion of our facility expenses, marketing, salaries, and website updates.

Nurse Navigation: Our oncology certified nurse navigator provides our navigation program. Because we exist in an under-resourced area, our navigator plays a crucial role in helping participants to decrease their distress by identifying the resources and supportive care needs they and their loved ones may face throughout their experience. These services exist to augment the support they receive in a clinical setting, and to access medical care. People hear of us by word of mouth, media, and from print collateral we have distributed throughout Southwest Colorado, through libraries, health-related agencies and businesses, and faith communities. We can be contacted via telephone, e-mail, through our website, or by personal office visits. We had 318 navigation and survivorship contacts in 2022, which is a slight increase over the prior year. Some of our expenses were books for our lending library, office and printing supplies, and a portion of our facility cost. The feedback we receive from the people we serve is consistently positive, as all these programs are filling a huge gap in our rural regions, particularly for the plethora of participants who are treated for their cancer, out of necessity, elsewhere.

Survivorship Programming - These programs provide no-cost emotional and social support for people with cancer and their families, and consist of educational workshops, healthy lifestyle classes, and social activities, which decrease social isolation and provide emotional support. In 2022, these consisted of a Survivor Self Care and Caregiver Virtual Retreat, a breast cancer networking group, a Managing the Cost of Care workshop, and 12 community hikes. Our programs combine social connection, companionship, and physical activities in nature all which create an environment that encourages participants to improve cancer patients, survivors, caregivers, and their families' quality of life as they are facing the multitude of issues presented by living with and beyond cancer. Form 990-EZ Part IV List of Officers, Directors: Toni Abbey RN OCN ONN-CG, in her roles as Executive Director and Oncology Nurse Navigator donates her time to CSC Southwest Colorado, which is not shown as an in-kind expense. These positions translate to an in-kind service of 2,040 hours at the Colorado Independent Sector's Wage of $31.51 per hour, equating to $64,280 in 2022. CSC Southwest Colorado had 32 volunteers who donated in-kind volunteer service of 620 hours, which translates to $19,536 at $31.51 per hour, according to the Colorado Independent Sector in 2022. Form 990-EZ is reviewed by the Finance Committee and the Board of Directors before filing. Governance Documents, organizational policies, and financial documents are located at the corporate office. The financial documents are also located on CSC Southwest Colorado's website and on GuideStar's website. Cancer Support Community Southwest Colorado requires that officers, directors, and employees complete a Conflict of Interest Questionnaire that management monitors and reviews on an annual basis.
