  • How to request records: Fax a letter seeking disciplinary documents to the Illinois Division of Professional Regulation at (217) 557-8073. Include your contact information, including e-mail address, as well as the license number, full name and city of the person about whom you are requesting records.
  • Lists of nurses disciplined since 1999 (NOTE: Nurses are included with other professionals regulated by the state.)

Serious Disciplinary Actions

States are required to report sanctions against health providers to the federal government. Below are the number of serious actions against registered nurses that Illinois reported in recent years, based on a ProPublica analysis of data from the Health Resources and Services Administration (see caveats).

This data should not be used to compare one state against another, because it may not be accurate and it is not adjusted for the number of nurses in each state. Another caution: Since September 2009, some states may have gone back and entered old actions they took against nurses. Check with each state before using these figures in a story.
