Issue:

VA staff mailed two copies of the Advance Directive to a Veteran (per his request) to his home. The staff unintentionally forgot to seal the envelope before mailing it. The US Post Office noted the envelope was not sealed so they indicated on the back of the envelope that it was mailed unsealed. When patient received the envelope he noted the comment on the back of the envelope by the Post Office and also noted that one of the Advance Directives was missing from the envelope. Veteran then called to notify Orlando VAMC about the incident. Staff determined that the personally identifiable information (PII) that was contained on the Advance Directive included the full name; SSN, DOB, address, etc.

Outcome:

04/28/15: The Incident Resolution Service Team has determined that the Veteran will be sent a letter offering credit protection services.
