The spouse of Veteran A returned to the Release of Information (ROI) office an opened envelope dated 11/24/14 via USPS with a copy of Veteran B's medical records. The spouse of Veteran A stated they did not request a copy of medical records and did not understand how they received someone else's information.
Outcome:
12/10/14: The Incident Resolution Service Team has determined that Veteran B will be sent a letter offering credit protection services.
