Issue:
Employee, from West LA VA, called clinic on 11/08/12 to report someone in the LA Clinic has been using the name and Social Security number of a Veteran who lives in Fort Smith. Patient was contacted by West LA employee and found at that time he had never been in the Los Angeles area. The person in LA is using Patient's A ID and Social Security number is a homeless man, no contact information is available. The VA ID picture which was taken in August, 2012, at the West LAVA is not Patient A. West LA employee wanted us to be aware that she has contacted the LAVA police and she will also contact IG. Patient A also presented to clinic on 11/7/12 with names and phone numbers of employees from West LAVA that has contacted him. Update: 11/13/12:The Veteran will receive a letter offering credit protection services.
Outcome:
Credit monitoring letter sent to Veteran and patient record was marked sensitive.
