Issue:

Patient A presented to the pharmacy booth on 11/06/12 concerned that the VA had the wrong address. He had some medications being mailed and thought that we were sending them to the wrong address. He gave employee a white slip that had been attached to the bag of medication that he had picked up from the Fayetteville pharmacy on 11/05/12. Upon inspection it was noted that the white tag was for a Patient B, not Patient A. The tag had the name and address for Patient B. Update: 11/09/12:Patient B will receive a HIPAA letter of notification.
