Issue:

Veteran A came to see the Patient Advocate and reported an alleged privacy violation. Veteran A reports that a Social Worker shared information with the Veteran's two step daughters, his now deceased mother in law, and also with Social Worker's mother and wife. Veteran A states this "was done with malicious intent." Veteran A reports this happened about a year ago and The Veteran just found out about it yesterday. Veteran A reports that the Social Worker spoke of his treatment related to PTSD and shared that he was being treated for anger management and then shared a word of caution about the Veteran's potential behavior. He reports this happened in a social setting. Update: 01/11/12:Veteran A will be sent a notification letter.

Employee was counseled by Supervisor regarding the giving of appointment information when not apart of your duties.
