Issue:

A VAMC employee provided 3 Veterans a fax number to submit applications to the VAMCs PPRTP program. The fax number was incorrect; applications were faxed by all 3 individuals to an office in the Illinois State office building. The local VAMC was contacted by the IL State Office. The faxed paper records were secured and picked up by the VAMC ISO within 2-hours of reporting. Information in the faxes contained full SSN, ful name, address, phone number and medical information. Update: 12/27/11:Three (3) Veterans will be sent letters offering credit protection services due to full name and full SSNs being exposed.

Outcome:

Education has been provided to staff. Fax number has been corrected on application packets.
