Issue:
West Palm Beach (WPB) Privacy Officer received notification from a Daytona Beach CBOC that one of their Veterans received information pertaining to a WPB veteran. Update: 10/05/11:One Veteran will be sent a letter of notification.
Outcome:
A meeting was held with the Supervisor and the employee. The severity of the potential of the situation was explained to the employee. The employee is relatively new to this department, and admitted to the human error. He said that he is trying to learn all aspects of the position, and that he was trying to multi-task and was moving too quickly. He did not indicate any malicious intent. After re-educating the employee, he confirmed that he will slow down and pay closer attention to his documentation in the electronic medical records. PO received the information from the CBOC representative. The information dates back to May 13, 2011 and contains nursing and physician discharge instructions (which includes a listing of the Veteran's medications), a clinician's opinion letter and a list of upcoming appointments. Unclear how this occurred . A discussion was held between the CBOC representative and the PO regarding the employees' responsibility in the appropriate handling of Veterans' health information. Copy of notification letter signed by the Medical Center Director was mailed to the patient. Redacted copy attached to close this ticket.
