Issue:
VA employee discovered that her husband was listed for a clinic appointment. She questioned who made the appointment and why, since her husband does not receive treatment at the facility. She questioned the access of her husband's medical record by her co-worker. Update: 10/06/11:The Patient will be sent a letter offering credit protection services due to full name, and full SSN being accessed inappropriately.
Outcome:
The Privacy Officer investigated and found that the employee's co-worker scheduled the appointments. Justification of access has been requested through the Medical Administration Service Supervisor. The appointments will be cancelled, since the Veteran does not seek treatment here. The Supervisor and the Service Chief have been briefed by the Privacy Officer. Guidance will be requested from Human Resources to take appropriate sanctions for inappropriate/unauthorized access by the co-worker.Appropriate disciplinary action will be taken by management. Letter offering credit monitoring has been signed by the director and mailed on 11/15/11. A redacted copy of the letter has been attached to close this ticket.
