Issue:

Patient A and Patient B had loose medication found by the mail distributor. The medication was returned by the mail distributor and was processed at the Dallas Consolidated Mail Outpatient Pharmacy (CMOP) only through verification. According to operation events the order was never packed. The medications bottles contained the patient name and type of medication. The cause of this type of packing error could be from staging super totes and regular orders at the same workstation The CMOP will properly package the items and reship to both patients. The technicians have been instructed to not process any orders in this manner. CMOP will monitor and enforce standing procedural guidelines and proper packing procedures with all technicians. Update: 04/06/11:Patients A and B will be sent a notification letter.

Outcome:

CMOP will monitor and enforce standing procedural guidelines and proper packing procedures with all technicians. Patient Notification Letters were mailed on 4/13/2011.
