Issue:
Patient A's medications were found loose by the mail distributor and returned to the Dallas Consolidated Mail Outpatient Pharmacy (CMOP). The mail distributor returned two bottles without patient documents or packaging. The patient stated the package was not open or damaged in any way but was empty of the medication. Sometimes any improperly sealed package, as a result of handling, can develop a sufficient enough seal to be delivered without any further problems. Dallas CMOP repackaged and reshipped the medication to Patient A. All packers/technicians will be retrained in proper packing procedures. Update: 04/06/11:Veteran A will be sent a notification letter.
Outcome:
All packers/technicians were retrained in proper packing procedures. Patient Notification Letter mailed on 4/13/2011.
