Issue:
Vocation Rehabilitation and Employment Counselor emailed a 28-1905 Authorization and Certification of Entrance or Reentrance Into Rehabilitation and Certification of Status application, containing Veteran As Name, address, and last four of his SSN to Veteran B. The Counselor contacted Veteran B and asked him to delete the content and the email from his inbox. Veteran B complied with the request and deleted the information.
Outcome:
08/27/14: The Incident Resolution Service Team determined that Veteran A will receive a general notification letter based on the data elements erroneously disclosed.
