Issue:

Veteran A visited the facility ER with his medications. Upon admission, his medications were taken by nurse and supposedly the Veteran signed a consent form. Not sure if staff verified Veteran A's address when receiving his medications. Veteran A stated he mentioned to nurse to "Do Not Mail" medication. He would pick up the medications after his discharge. The medication slip was forwarded to Pharmacy, who in turn mailed the Veteran's medications to address on file, at which he no longer resides. The medication package was opened by his ex-girlfriend, who notified him. the medication was used for a protected 7332 condition. Currently, ER does not have a signed copy of the consent form signed by Veteran or nurse. The Privacy Officer (PO) is working with information Security Officer (ISO). The Patient Advocate Office will investigate with involved services). More information will be forthcoming.

Outcome:

07/14/14: The Incident Resolution Service Team has determined that Veteran A will be sent a HIPAA notification letter due to Protected Health Information (PHI) being disclosed.
