Issue:

Veteran A requested a copy of his claim file (c number, not SSN). The copy was addressed to Veteran B in error. Veteran B returned the documents to Leavenworth VSO who returned it to the Regional Office. The file contains names of the Veterans ex-wives and children so number of affected individuals has yet to be ascertained.

Outcome:

10/28/14: The PO expects to receive the file in a couple of days and then will be able to determine the number of affected individuals. 11/10/14: The Incident Resolution Service Team has determined that Veteran A will be sent a letter offering credit protection services. There is no way to contact the 3rd ex-wife.
