Issue:

Veteran A contacted the Privacy Officer (PO) to advise that Veteran B called him stating that he had received records on Veteran A. Veteran B has never received his records either. Veteran A and B's records were sent out the same day. Veteran B received the records with a cover letter with Veteran B's information on it; however the records were Veteran A's. It was never verifed if Veteran B ever received his records, but at time of call, he had not. Veteran A requested to be sent another copy of his records. Update: 12/06/12:Veteran A will be sent a letter offering credit protection services.

Outcome:

Both PO and Chief HIMS have spoken to both Veterans and apologized. PO has stressed the importance to the HIMS Chief, of checking all information that is going into the envelopes that are being mailed out to Veterans. Check and Double check process.
