Issue:

The Privacy Officer received letter today from Veteran A who stated that he received Veteran B's armband during his October 2012 Emergency Room visit. Veteran A named Veteran B that was on the Armband. There are 21 Veterans names matching the name that he identified. The Privacy Officer has requested assistance from the Business Office and Information Resource Management to narrow the search. The letter implies that he only had the armband on a few minutes when the other Veteran was called and no one answered. The reporting Veteran's driver noticed the armband was incorrect. Update: 12/10/12:Veteran B will be sent a letter offering credit protection services due to full name and SSN being disclosed.

Outcome:

This issue was discussed with the specific MAA staff and the Assistant Chief Business Office. Education has been provided to all Medical Administrative Assistants and Administrative Officers of the Day.
