Issue:
VA Illiana Health Care System Clinic Clerk inadvertently placed Veteran B's order of "Return to Clinic" letter with Veteran A's letter and mailed it. Veteran A contacted VAIHCS when they received the information and stated that they had shredded the letter belonging to Veteran B in their home paper shredder.
Outcome:
01/28/14: Veteran B will be sent a letter offering credit protection services.
