Issue:

A folder of pending consults for scheduling was pushed under the door of a Dorn VA provider. That provider was out for the day and an Intern entered that office with a Veteran. The Intern picked folder up from the floor and placed in a chair. Veteran set his paperwork over that folder and picked up everything together upon departing from the appointment. After staff noticed it was missing they contacted the Veteran to determine if he had it. Veteran confirmed the folder was at the bottom of his paperwork. He did not notice until the call from Dorn VA staff. Veteran mailed folder in its entirety via USPS next day delivery on 01/12/15 and Dorn VA provider received package on 01/13/15.

Outcome:

01/14/15: The Incident Resolution Service Team has determined that five Veterans will be sent letters offering credit protection services.
