Issue:

A Veteran's wife received a Fee letter that should have been sent to another Veteran (see PSETS 96342). The wife and her husband (a Veteran registered at our facility) went to our ROI unit and requested information that had been scanned in VistA imaging. That scanned information was on the Veteran who the Fee letter should have been sent to. The scanned information was the Veteran name, birth date, home address, medications, radiology report information, diagnoses and treatment. The information was discovered when the Privacy Officer was meeting with the wife and her Veteran husband regarding PSETS 96342 regarding the Fee letter.

Outcome:

10/29/13: Veteran B will be sent a letter offering credit protection services.
