On February 24, 2014, we mailed a notification letter to Veteran A, and attached to his letter was the Rating Decision belonging to Veteran B. Veteran A has been contacted and is in the process of returning the misdirected mail. Veteran B's Rating Decision contained his name and full SSN.
Outcome:
02/28/14: The Incident Resolution Team has determined that Veteran B will be sent a letter offering credit protection services, as his full SSN and other information were disclosed.
