Issue:

A Compensation and Pension employee contacted the Privacy Officer to report a privacy incident. The Veteran presented with another unrelated matter. When the Veteran began looking through his paperwork, he discovered that he had two appointment letters, one appointment letter contained his name, and the other contained another veterans name. The Veteran recalls receiving some documents from the lab. The Veteran visited Chain of Custody and MRI today. He was not scheduled for a Chain of Custody appointment, but he was contacted on Monday to come in for random screening since he is on a narcotics contract.

Outcome:

07/30/13: One Veteran will be sent a HIPAA notification letter due to Protected Health Information (PHI) being disclosed.
