Issue:

A Veteran called the Pharmacy Supervisor who then reported to facility Privacy Officer (PO) that this Veteran had received another Veteran's leaflet/medication insert along with correctly mailed prescription and mailed information for himself. The Pharmacy Supervisor is in the process of attempting to recover the incorrectly included leaflet. The Veteran thinks he has already shredded it at home. Fact-finding and remediation/corrective action assigned for follow-up.

Outcome:

02/27/15: The Incident Resolution Service Team has determined that Veteran B will be sent a HIPAA notification letter due to Protected Health Information (PHI) being disclosed.
