Lab letters for 3 individuals were accidentally combined and mailed to a 4th individual. Patient had letters for some time, due to move, before he could bring them back in. Letters were dated Nov 17-18, 2014, returned on December 5, 2014.
Outcome:
12/05/14: The Incident Resolution Service Team has determined that three Veterans will be sent a notification letter.
