Findings:During the review of the facility Unusual Occurrence Report dated 4/29/13, the report indicated Discharge Nurse A (DCN A) gave an After Care Instruction Sheet and a sheet of patient labels (pre-printed labels that contain patient information) to Patient 1. The information given to the wrong patient contained the following: Name, date of birth, medical record number, insurance information, and the surgery date. The information was returned to the facility.DCN A received additional training and counseling.
