Based on record review, the hospital failed to safeguard Patient 1's Protected Health Information (PHI) which resulted in an unauthorized person receiving his PHI.Findings:During the review of the facility Unusual Occurrence Report dated 4/29/13, the report indicated Billing Coder A sent Patient 1's bill to the wrong patient. The information on the bill contained the following: Patient name, address, account number, procedure done, and billing cost.Billing Coder A received additional counseling and patient privacy training.
