Issue:
Patient A received a prescription package intended for Patient B. CMOP Dallas discovered the error internally and confirmed with a phone call to Patient A. Item for Patient A was scanned at packing but item for Patient B was placed in the bag. Both items were part of a Manual Toteless order which appear to not have been removed from zipper bag after pharmacist verification. Patient A returned the medication to the medical center. Patient B's name and type of medication was compromised. The Packer will be counseled on error and Pharmacist will receive review of manual toteless verification procedures. Update: 09/01/11:Veteran B will be sent a notification letter.
