Issue:

It was reported to the Dallas Consolidated Mail Outpatient Pharmacy (CMOP) that Veteran A received two prescription packages but only one was the medication he ordered. Both packages had Veteran As mailing address, but one package was for Veteran B. Veteran A lives in Oklahoma and Veteran B live in Texas. Veteran As order was processed on a Friday and Veteran Bs order processed on the following Monday. Since there is no cold chain items shipped on Friday, both orders shipped on Monday. According to documented processing events, neither order showed an anomaly. No rescans or reprints noted. It is unclear how a second address label for Veteran A was available to place on the container for Veteran B. Veteran A returned the medication to the medical center. Veteran B's name and type of medication was compromised. The CMOP will review their cold chain process with employees to ensure it is being followed correctly. Update: 09/01/11:Veteran B will be sent a notification letter.
