Issue:

A nurse at a contracted Community Based Outpatient Clinic (CBOC) gave labs to a Veteran. A provider at the contracted CBOC reviewed the labs with the Veteran. Later in the evening the provider realized the labs reviewed with the Veteran belonged to another Veteran. The associate called the Veteran (who the labs were discussed with) and she stated she had shredded the labs when she realized they were not her labs. The correct lab results were given to the Veteran over the phone. The medication that had been ordered was cancelled and this Veteran had not yet received the medication.

Outcome:

11/22/13: The Veteran will be sent a letter offering credit protection services, as his full SSN was disclosed.
