Issue:
An appointment list was inadvertently handed over to Veteran A along with some other informational documents at the time of clinic check-out. The employee who handed over the appointment list realized her mistake and immediately contacted Veteran A to return the documents, however, Veteran A had already made it home and agreed to shred the documents.
Outcome:
12/01/14: The Incident Resolution Service Team has determined that there was a policy violation. While there was an unauthorized access/disclosure of data, it has been determined that the incident has a low probability of a risk of compromise.
