Report ID: PSETS0000096984, U.S. Department of Veterans Affairs

Reported Entity: PHOENIX AZ - 345

Issue:

CityScape, found some financial documents left behind when the Regional Loan Center moved out of their building last March. There were a total of seven documents which included, Certificate of Eligibility (COE) and Indebtedness Letter applications with PII on them in a red shred envelope. The six of the documents were verification of VA Benefit, which contained; the veterans name, address, DOB, social security numbers, benefit amount, and if he or she had a debt with the VA. The seventh document contained the Request for a COE, which contained the following information. The veterans name, address, DOB, social security numbers, phone number, Dates of Service, Branch of service, and benefit amount. It is believed that, since they were in a shred envelope, were relatively sure that action had already been taken on the requests. If not, the lender would have followed up and asked again by now.

Outcome:

11/18/13: These documents were left out of VA control for months. All seven individuals will be offered credit protection services.
