Issue:

Patient A's medication was found loose in the bottom of the mail distributors return bag. It was returned without packaging and patient documents. The package was recalled and returned to the CMOP. Noted upon inspection of the package was a complete seal, but the bottom end of the package was open. The mail distributor stated the package arrived at the facility damaged. A box cutter is used when opening the stock boxes for all the packages used by the CMOP packers. This package was mistakenly cut at that time and due to the handling process the opening increased in size allowing a bottle to fall out of the package while in transit. The medication and the package were returned to CMOP. The CMOP will develop a new procedure for opening stock boxes of packages used by the packers. Update: 04/04/11:Patient A's information that is missing included his full name and medication inforamtion. Pateint A will receive a letter offering credit protection services.

Outcome:

The packaging error has been reported to Medline for investigation and corrective actions. Patient Notification Letter mailed and copy attached.
