Issue:
Patient A's medication was found loose by the mail distributor. Patient's name and type of medication was compromised. The medication was returned to Dallas CMOP without the patient documents and package. CMOP investigation concludes that the technician did not complete the packing process as instructed. Dallas CMOP properly repackaged the medication with patient documents and reshipped. The employee is a new hire and will be retrained in proper packing procedures. Update: 04/04/11:Patient A's documents included his name and medication information. Patient A will reecive a letter of notification.
