Issue:

Veteran A was handed a copy of Veteran B's check-in ticket by the provider along with Veteran A's radiology order. Veteran A checked out and the clerk noticed the check-in ticket did not belong to Veteran A. It is not clear if Veteran A looked at the check-in ticket or not. The information on the check-in ticket was last name, first name, last four digits of the Social Security Number, date of birth, check-in time, appointment time, provider time, appointment type, blood pressure, pulse, temperature, respirations, and diagnoses for Veteran B.

Outcome:

04/22/15: The Incident Resolution Service Team has determined that Veteran B will be sent a HIPAA notification letter due to Protected Health Information (PHI) being disclosed. 04/22/15: Update: Full DoB was disclosed as well. The Incident Resolution Service Team has determined that Veteran B will be sent a letter offering credit protection services.
