Issue:
A Veteran called the hospital and said that he received a paper containing another patient's name. He received the paper either at the main hospital or at a CBOC. Veteran said that he would not mail the paper back to the PO; however, he would take the paper to the Clinic. Identity of the Veteran whose privacy was breached is unknown at this time.
Outcome:
11/05/13: The Patient called the Privacy Officer and informed her of name of the Veteran and the PHI which was included in the mailing. The paper included the Veterans full name, full SSN and medications list. A letter offering credit protection services will be sent to the Veteran.
