Issue:
Two patients slides and paper work that were packaged for shipping was left on top of the UPS box located on the first floor. The packages were not picked up by UPS. The package with the patients information is missing. the information contained the Veterans full name, full SSN, medical information, and other PII.
Outcome:
03/31/14: The Incident Resolution Team has determined that both Veterans will be sent letters offering credit protection services.
