Issue:

Loose prescriptions were returned from the mail distribution center as a result of improper packaging. The Dallas Consolidated Mail Outpatient Pharmacy (CMOP) employee did not complete the packing process as instructed . As a result of an improper seal on the bag some of the contents were able to fall out. The patient's (Patient A) name and type of medication was compromised. CMOP investigation concludes that this was a CMOP packing error. The employee is a new hire and will be retrained in proper packing procedures. Update: 03/02/11:Patient A will receive a letter of notification.

Outcome:

Patient Notification Letter mailed on 3/23/2011. CMOP packer has been counseled and retrained in proper packing procedures. Please close this ticket.
