Issue:

A labeled prescription was found without proper packaging in a gaylord (a big wheeled basket used by the mail distributor to hold packages) at the mail distribution center. Dallas Consolidated Mail Outpatient Pharmacy (CMOP) operation events for this order indicates the medication was never sent through the packing process. The mail distributor returned the prescription to CMOP. The patient's (Patient A) name and type of medication was compromised. CMOP investigation concludes that proper packing procedures were not used. All packers will be retrained in proper packing procedures. Update: 03/02/11:Patient A will receive a notification letter.

Outcome:

Patient Notification Letter mailed on 3/23/2011. CMOP packer has been counseled and retrained in proper packing procedures. Please close this ticket.
