Based on record review, the facility failed to protect the Private Health Information (PHI) of one Patient (A.) This failure caused a breach of Patient A's PHI.Findings:The facility Breach Report, dated 2/26/13, was reviewed on 2/27/13. The report indicated Patient Access Representative 1 (PAR 1) faxed Patient A's PHI to a wrong fax number. PAR 1 indicated, in the facility investigation, that the extended care facility had given her the wrong number to fax information to. The fax contained the following information: Patient A's name, date of birth, test to be completed, and the name of the extended care facility that the patient lives in. The reports were destroyed.The facility investigated the breach and provided training and counseling to PAR 1.
