Issue:
Based on interview and record review, the hospital failed to safeguard three patients' (Patient A, B and C) Protected Health Information (PHI). This resulted in the unauthorized access of Patient health information. Findings:The facility Breach Report, dated 4/26/13, was reviewed on 2/26/13. The report indicated Phlebotomist 1 took her niece to work with her. The niece accompanied Phlebotomist 1 to the emergency room while she was drawing blood from Patients A, B and C. Phlebotomist 1 did not have facility approval to have her niece accompany her to work. The niece did not have privacy training, nor were consents obtained from Patients A, B or C. Phlebotomist 1 was counseled and educated on patient privacy.
