Issue:

On 02/09/15 a Medical Technician printed and gave a medication list to Veteran A. The receiving patient called to notify the Medical Technician that the medication list belongs to Veteran B. PII identified on the medication list includes full name of the Veteran full SSN. The Medical Technician followed up with the receiving patient to retrieve the medication list. Patient was unable to forward the medication list back to the VAMC however, he assured the Medical Technician that he would shred the paper.

Outcome:

02/10/15: The Incident Resolution Service Team has determined that Veteran B will be sent a letter offering credit protection services. 02/26/15: Update: Based on new information from the Privacy Officer, The Incident Resolution Service Team has determined that Veteran B will be sent a HIPAA notification letter.
