Issue:

Patient A received Patient B's appointment letter. Letter was mismailed (double stuffed). It contained Patient B's full name, last four of their SSN, Protected Health Information (PHI). It was out of VA control for over 72 hours, involves one patient and we were unable to recover the letter. Update: 05/02/11:Patient B will receive a letter of notification.

Outcome:

Provided privacy training to employee and section staff so that this will reduce the chances of happening again.
