Issue:
A complaint from Veteran A was forwarded to the facility Privacy Officer (PO) noting that on a previous visit to Emergency Department, a nurse had incorrectly handed Veteran A a copy of Veteran B's printed discharge instructions with medication reconciliation information, diagnostic-medical information, SSN, full name, and DOB. Veteran A, who had received the incorrect paperwork then returned the printed, incorrect paperwork on Veteran B to facility Patient Advocate who turned in the incorrect information on Veteran B to facility PO.
Outcome:
05/18/15: The Incident Resolution Service Team has determined that Veteran B will be sent a letter offering credit protection services.
