Issue:

On 2/5/14 Veteran #1 presented to the Cardiologist Clinic for an appointment that he thought he had that day. The Veteran arrived to the clinic holding another Veterans appointment letter. Veteran #1 did not realize that he received another Veterans appointment until the Program Analyst discovered that his appointment was scheduled the following week and not today. The Program Analyst immediately noticed that the letter did not belong to Veteran #1 when he handed her the letter. She asked Veteran #1 if there were other letters in the envelope thinking perhaps two were put in the same envelope by mistake; however, Veteran #2 was contacted by phone and he stated that his mailman delivered him the correct letter, and that no other letters were in the envelope. Both patients were seen in the vascular clinic. The Privacy Officer will provide an update.

Outcome:

03/03/14: Veteran #2 will be sent a notification letter.
