Issue:
The Release of Information (ROI) clerk faxed information to an outside provider. Included with the correct information was another Veteran's information. The information included full name and last 4 digits of the Social Security Number. Once she realized she had made the error, she called the outside provider and informed him to shred the information that was given to him in error.
Outcome:
06/22/15: The Incident Resolution Service Team has determined that there was a policy violation. No data breach has occurred.
