Issue:

Veteran A's VA Form 10-7079 "Request for Outpatient Services" was inadvertently placed into Veteran B's Fee Basis appointment packet. Veteran B returned the 2-page form to the nearest Community Based Outpatient Clinic (CBOC), who in turn notified the Privacy Officer (PO). The form was dated 06/10/13, and likely discovered as the Veteran was double-checking his paperwork after receiving in the mail several days ago. The form contained Veteran A's last four of SSN, home address, home phone, and a one-line description for which services were requested: "GI - Loss of Weight."

Outcome:

06/21/13: Veteran B will be sent a letter offering credit protection services.
