Based on record review, the facility failed to protect the private health information (PHI) of Patient A. This failure caused a breach of Patient A's PHI.Findings:The facility Breach Report, dated 4/8/13, was reviewed on 4/9/13. The report indicated Care Management RN 1 sent a fax containing PHI information of Patient A to the wrong fax number. The fax contained the following information: Patient A's name, date of birth, age, sex, address, phone number, Social Security Number, marital status, health information, medical record number, and account number. It also contained the next of kin's name address and telephone number.The facility was unable to determine who the information was sent to.The facility investigated the breach, and provided training and counseling to Care Management RN 1.
